Case Information
To view your payment history and case information, you will need to create an account profile to
access the application. Account creation is a four step process. Please refer to the
following sections of this FAQ for details on how to complete each step of the account creation process.
Please note that your information will only be saved when you complete all of the steps.
If at any time you stop the creation process, you will need to start over from Step 1.
How do I ...
Create a new account profile?
To create your account you will need to enter specific information at each step of the process.
You will need to provide the following information:
- Step One
- Case Number
- PIN
- Step Two
- Date of Birth
- Step Four
- Your email address (a valid and current email address)
Complete Step 1?
- Enter your case number (if you have more than one case number, choose one).
You only need to create one account to view the information for all of your cases.
- Enter your Case PIN Number into the PIN field.
- If you do not remember your PIN, Click on the send me
my PIN link located in the Forgot Your Case Number or PIN? section
- Click the Continue button
You will have a limited number of attempts to enter the correct information.
Complete Step 2?
- Enter your Date of Birth, use the date format of MM/DD/YYYY i.e., 01/01/1900
- Click the Continue button

Complete Step 3?
The application generates a Username for you that will be based on your first and last name,
separated by a "." and may be followed by a sequence number. For example, if your name is
john smith your username will be john.smith. If a username of john.smith already
exists in the system then your username will be john.smith1 and so on.
Please remember and record this information because you will use it, along with the
Password you create, to log into the application. Your username
must be entered in all lower case and the password is case sensitive.
- Enter a password in the Password field and the same password in the Confirm Password field
- Passwords must meet the following criteria:
- At least one upper case letter (A Z)
- At least one lower case letter (a z)
- At least one number
- The password must be between 8 and 30 characters
- Your name or username CANNOT be part of the password
- The Password cannot contain any special characters. Examples of special characters include ! @ # $ _.
Passwords are encrypted and the actual password cannot be seen or decrypted. A
temporary password will be issued if you forget your password.
Complete Step 4?
- Enter your personal email address

Be sure to use a valid and active email address that belongs to you. If the email is not valid
then we will not receive your request. Please do not share your email account login information with others and for your security
do not use a shared email account.

If you do not have a valid email address, there are several sites that offer free email services (gmail.com, yahoo.com, msn.com, etc).
- Please check the spelling of your email address.
- Your email address will allow you to retrieve your Username or assign a temporary password should
your forget them.
- Enter your primary phone number and/or a secondary phone number

Your telephone information is optional, but providing your telephone number gives OCSS another means of contacting you.
- Please select and answer 5 security questions

The same question cannot be selected more than once.
- Be sure to remember the questions and their corresponding answers.

The application will require you to answer a security question when requesting the retrieval of your
Username or issuing you a temporary password if you have forgotten your Password.
The information you provide is private and we do not shared this information.
Complete account creation process?
The Account Creation Confirmation page displays the information that you have entered
in Steps 1 through 4. Before clicking on the Confirm button,
please verify the data.

If any information requires updating, click the Edit button in
each section to edit that information.

Click the Cancel button to undo your changes if you have made
changes before clicking the Update button
- When you are finished reviewing or making changes, click the Update button
Click on the Confirm button to be taken to the Login page where you can now to log in to the application
